New Patients

After requesting your first appointment through the Patient Portal, you will receive a confirmation email with instructions to access your account. Once your appointment is confirmed by our staff, you will receive a second email containing the video appointment link and a prompt to complete your intake forms. Please ensure that all required forms are submitted at least 48 hours before your scheduled visit to avoid any delays.

Typical intake forms include:

  • Patient Intake Form
  • Consent for Treatment
  • Telehealth Consent
  • Notice of Privacy Practices (HIPAA)
  • Patient Financial Responsibility Agreement
  • Good Faith Estimate (No Surprises Act)
  • Authorization for Release of Information (if applicable)

Established Patients

If you’re scheduled for a follow-up, you may receive a brief screening form relevant to your ongoing care prior to your visit. These forms are also sent through the Patient Portal and can be completed from any device.

Examples include:

  • PHQ-9 for depression
  • GAD-7 for anxiety
  • ASRS-v1.1 for ADHD
  • Other forms based on clinical needs
